Effective on 1/18/24, Concentric Psychiatry will no longer be providing mental health services. It has been a great pleasure providing for your mental health care needs. I recommend looking for a new psychiatric provider as soon as possible to avoid any lapse in care.
Your medical records are confidential, and a copy can be transferred to another provider or released to you or to another person you designate only with your permission. Please complete a Release of Information form and return it to 166 Hargraves Dr., Suite C400-130, Austin, Tx 78737 as quickly as possible so we may prepare your records for you or your new provider. Your records will continue to be on file at my former office.
